What is Care Partners at Home?
Care Partners at Home is a healthcare company located in Irvine, California that focuses on senior care. This means that they provide caregivers, transportation services and more, but more on that later. It was founded in 2016 by Randy Platt and Rodney Burris. Their career long-vision was finally made reality, and now halfway through 2018 their services range from San Luis Obispo to Yorba Linda, California, covering almost all of Orange County. While this does mean that Care Partners at Home is still technically a start-up, they have been extremely successful in differentiating themselves from their competition with their company values of Compassion, Empathy and Love. Below is a better illustration of the services the company offers.

But what do all of these mean? In-Home Care regards the company’s ability to provide personal care, light housekeeping, meal preparation, medication reminders and disease-specific management. Companionship regards engagement activities, documenting life stories, outings, social visits and providing a travel companion. Physician Services regards care coordination, concierge care, house calls, telehealth and medication management. Care Management Services regards in-home assessments, medical coordination, records management, social work and caregiver oversight. Transportation regards social outing, medical or personal appointments, and relocation and/or transition. Finally, Comprehensive Assessment regards a customized care plan and disease-specific management. These services are invaluable to the population we serve, and more often than not we become like family members to our clients, and not just a business.
In addition to the Care Partners at Home services, the company has just merged with two others; Global Transitional Care (GTC) and Personalized Senior Placement.

This will allow Care Partners at Home to provide more services than before, once the merger is officially announced later this year.

Organizational Structure of Care Partners at Home

Above is the hierarchical structure of Care Partners at Home. As you can see above, both Mr. Platt and Mr. Burris sit at the top of the structure, since they are the founders as well as the CEO and COO respectively. Following them is my supervisor, Yolanda Lawler, the Executive Director. She fulfills many duties within the office including: overseeing scheduling, maintaining client accounts and some minor marketing just to name a few. I sit underneath Yolanda as the intern, since she is my supervisor. My duties include assisting Yolanda with various projects like client accounts and marketing, as well as completing projects assigned by Mr. Platt and Mr. Burris. Also underneath her is the rest of the office staff, who all share the same hierarchical standing underneath Yolanda. Monique, the Client Care Coordinator, is responsible for scheduling caregivers and matching the right caregiver with the right client. Rebecca is responsible for recruiting new caregivers, as well as running orientations for them. Mark, the Business Office Manager, is responsible for managing the daily goings-on of the office, from scheduling meetings to maintaining office supplies and equipment. Finally, there’s Ben, the Director of Business Development, who also does case management, as well as the Life Care Manager Kathy, who is responsible for creating and maintaining the customized care plans that Care Partners at Home provides to Caregivers.
The Senior Care Industry
According to franchiseopportunities.com, the senior care industry has exploded over recent years due to a massive demand for caregivers for people over the age of 65. In the last 10 years, the population has increased 28%, the biggest that age range has ever seen. By 2060, the population is supposed to reach 21.6% of Americans, so the need for the industry to grow is there.

In addition to being a growing market, it is also highly competitive, with clients having to choose from a range of different companies. Care Partners at Home distinguishes itself from the competition with its mission statement, stated in the next section, and by the nature of its employees and management staff. Every employee seems to care and understand everything that their client is going through, and that helps put families and potential clients at ease.
Who do we serve?

Care Partners at Home serves seniors and their families living in Orange County. This ranges from San Clemente and Dana Point, all the way to Los Alamitos and Seal Beach, which border Los Angeles County. The company’s mission statement is, “We will create deep meaningful relationships with our clients, their families, our associates, and partners to establish high energy environments focused on true person centered caring, while increasing the quality of their lives, and ours, within every interaction. Great page! You’ve done a great job describing and explaining the company and your involvement. Your charts and pictures support your text well and are easy to understand. This is a much needed service now that people are living longer and longer. I think the mission statement is something that can set them apart and I especially appreciate the company’s focus on the relationship aspect of the service. I look forward to reading more about your internship throughout the summer.
